Motixafortide
Motixafortide is a potent, long-acting CXCR4 antagonist that stimulates the release of hematopoietic stem cells from the bone marrow into the peripheral blood. It is FDA-approved for use in combination with filgrastim to mobilize stem cells for collection and autologous transplantation in patients with multiple myeloma.

02Mechanism of action
CXCR4 Antagonism
Motixafortide binds to the CXCR4 receptor on hematopoietic stem cells, blocking its interaction with the SDF-1 (CXCL12) ligand.
Stem Cell Mobilization
Blocking this interaction disrupts the retention of stem cells in the bone marrow, causing them to migrate into the peripheral blood.
Enhanced Collection
This allows for a higher yield of stem cells during apheresis, improving the success of autologous stem cell transplants.

Significantly improved stem cell mobilization in multiple myeloma patients.
The Phase 3 GENESIS trial showed that motixafortide plus filgrastim enabled 92.5% of patients to collect the target number of stem cells in up to two apheresis sessions, compared to 26.2% with filgrastim alone.
Rapid mobilization allowing single-session collection.
The majority of patients in the GENESIS trial achieved the target stem cell yield in just one apheresis session, reducing the burden on patients and healthcare systems.

Prolonged receptor occupancy.
Animal models demonstrated that motixafortide has a much longer half-life and receptor occupancy compared to earlier CXCR4 antagonists like plerixafor.
Potential in solid tumors.
Preclinical studies in pancreatic cancer models suggest that blocking CXCR4 with motixafortide can alter the tumor microenvironment and enhance the efficacy of immunotherapies.
